A rakghoul serum was an ingredient that was able to cure the plague of rakghouls which, after infecting an individual, turned the person to an Rakghoul.
In the Jedi Civil War, a test sample from the serum had been developed by Republic scientists, and duplicated several thousand times in order to heal the Outcasts in the restoration of the Republic’s Taris. The Republic was home to an bases located in Taris’s Upper City as the main center of the restoration process as well as the site in which the scientist had perfected the cure. However there was a problem. The Sith army was aware that the world was in trouble and attacked. After shoring the Republic out the base, they seized control of their base. They also took over the base. troops of Sith took samples of rakghoul serum for patrolling in the Undercity looking for the destroyed Republic escape pods that were escaped from their destruction of the Endar Spire. The Sith encountered so many times with the rakghouls that the had run out of serum at the time Revan was able to enter the Undercity but certain troopers who died were carrying serum samples on their bodies.
While he was on his way to get out of Taris, Revan found a sample of the serum in the Undercity and passed it on the Zelka Forn who was who was the Upper City doctor. He made the serum and distributed it to everyone who was infected and also gave a substantial amount to the healer of Outcasts Esala who was able to use it to treat Infected Outcasts on their way towards The Promised Land. When they arrived at their destination, the Promised Land, it was not what they had hoped for. The infrastructure, buildings and defenses had all been destroyed with the Sith. With no protection, the rakghouls were able to attack Outcasts however, the spread of the disease was thwarted with the help of the serum. The rakghouls grew but, over the course of several generations, they developed resistance to serum. There was no defense from the spread of rakghoul virus, and with an ill-fated and bombed-out world all around them the Outcasts were eventually eradicated out.
In 3643 BC The rakghoul serum was rediscovered by pirates who discovered an unopened bottle of the serum in Zelka Forn’s abandoned second medical facility located in the Undercity. The serum was then given to an unnamed Jedi and then was able to get into the hands of the Republic doctor. But the rakghouls were immune to the serum before the Outcasts were eliminated, which means that it wasn’t as efficient three hundred years after. A new serum had been created in order to be effective, and to do it to be effective, a person must be infected to create new antibodies. These can then be examined and synthesized. The infection that was controlled was passed to the subject and the antibodies were analyzed and a new, enhanced serum was developed and then, again it was created in large quantities.
